These terms apply to all Playmaker Agency programs, including holiday camps, term programs and free trial sessions (together, "programs"). By registering a participant for any program, or by requesting a free trial session, the parent/guardian agrees to the following terms on behalf of the participant:
I acknowledge that participation in football training and related physical activities involves inherent risks, including but not limited to physical injury, illness, or accidents. I voluntarily allow my child to participate and accept these risks.
In the event of injury or illness, I authorise Playmaker Agency staff to seek medical treatment or emergency services if deemed necessary. I accept responsibility for any associated medical costs.
I confirm that my child is physically fit to participate and has no medical condition that would prevent safe participation, unless disclosed before their first session.
To the extent permitted by law, I release and hold harmless Playmaker Agency, its coaches, contractors, and affiliates from any liability for injury, loss, or damage arising from participation in any program, except where caused by gross negligence.
I understand that unacceptable behaviour may result in removal from the program without refund.
Playmaker Agency is responsible for participants during scheduled session times only. I am responsible for my child before a session starts and after it finishes, including arriving on time for drop-off and collecting my child promptly at the end of each session.
I consent to photos and videos of my child being used for marketing and promotional purposes by Playmaker Agency unless I request otherwise in writing. This applies to all programs, including free trial sessions.
Free trial sessions are available to new players only, limited to one per player, and subject to availability. Submitting a free trial request does not guarantee a place: a trial session is only confirmed once a Playmaker Agency coach replies to confirm it. These terms apply in full to any trial session attended.
Playmaker Agency camps are delivered in affiliation with AFA Internacional. Term programs and free trial sessions are run independently by Playmaker Agency and are not affiliated with AFA Internacional.
I acknowledge I have read and agree to the refund policy that applies to the program I have registered for.
By completing registration, or by submitting a free trial request, I confirm I have read, understood, and agree to these terms.
