By completing registration and payment for Playmaker Agency soccer camps, the parent/guardian agrees to the following terms on behalf of the participant:
I acknowledge that participation in soccer training and related physical activities involves inherent risks, including but not limited to physical injury, illness, or accidents. I voluntarily allow my child to participate and accept these risks.
In the event of injury or illness, I authorise Playmaker Agency staff to seek medical treatment or emergency services if deemed necessary. I accept responsibility for any associated medical costs.
I confirm that my child is physically fit to participate and has no medical condition that would prevent safe participation, unless disclosed prior to the camp.
To the extent permitted by law, I release and hold harmless Playmaker Agency, its coaches, contractors, and affiliates from any liability for injury, loss, or damage arising from participation in the camp, except where caused by gross negligence.
I understand that unacceptable behaviour may result in removal from the camp without refund.
I consent to photos and videos of my child being used for marketing and promotional purposes by Playmaker Agency unless I request otherwise in writing.
I acknowledge I have read and agree to the refund policy outlined at registration.
By completing payment, I confirm I have read, understood, and agree to these terms.
